Bridging the Gap
In addition to the stabilizing services and transformational programming offered at our East Nashville Community Resource Hub, we offer housing solutions that bridge to permanent housing.
Mobile Housing Navigation Centers
Community Care Fellowship’s Mobile Housing Navigation Centers (MHNC) are developed in partnership with Metropolitan Government of Nashville’s Homeless Impact Division, non-profit organizations, and local congregations. MHNC’s provide temporary housing for our neighbors experiencing homelessness for up to 120 days, with the goal of connecting each guest with income, addiction and mental health services, permanent housing, and a strong support network.
Guest referral to one of CCF’s MHNCs is handled by the Metro Homeless Impact Division, which utilizes the Coordinated Entry process to provide hyperlocal referrals off the By Name List.
Mobile Housing Navigation Centers complement CCF's Rapid Rehousing Program. We understand that many people experiencing homelessness avoid entering the shelter system for a multitude of reasons. These centers provide a bridge between encampment and permanent housing by not only providing a place to live, but also equipping residents with the necessary skills, resources, and support to be successful.
Rapid Rehousing Program
Thanks to partnerships with local churches and others organizations, we provide people with immediate support—especially in emergency situations. We know that people can’t thrive in crisis mode, which is why the goal of our Rapid Rehousing Program is to move them into permanent housing in less than 90 days.
